Things are changing, Paul. UK is leading the way in proving powerful antibiotics are unnecessary with transperineal biopsies.

For what it's worth, the first time I had a transperineal prostate biopsy at a London NHS hospital in late 2022, I was given a large antibiotic pill to take afterwards, just as prophylactic cover. But when I had a second TP one in early 2024, they didn't bother with the antibiotic, saying they had now decided it just wasn't needed. That probably says something about the relative risks of the perineal and rectal approaches.

Tried to fill out the survey, but one of the required questions reads "If you had a trans perineal... I'd the answer is you did not, there is no choice ,and it is required, so I could not submit it.
